Coventry/Carelink offers plans for businesses from 2 employees on up.  They offer a variety of plans with varying deductibles, coinsurance percentages, prescription drug plans, and office copays. Groups over 50 may custom-design their own plan.

Coventry Healthcare group coverage includes HMO, POS and PPO plans, as well as consumer-directed options. Options for vision, dental, durable medical equipment, prescription coverage, and other options are available. Our consumer-directed plans -- Coventry Consumer Choice (C3) -- offer many choices of deductible and personal health account (HSA, HRA, and FSA) choices.

To get quotes for a group, the group must complete a few simple steps.  Each employee that wants to be included in the plan must fill out a universal medical appraisal form, and must complete all information on the three page form.  Any employee who wishes to waive coverage must complete the first page of the form and sign at the bottom in the waiver section.  

Once we receive these items, we will submit the group to Coventry for a firm, underwritten quote.  This typically takes 1-3 weeks, depending on how complete the information is.When the quotes come in, we can meet with you to go over all of your options and help navigate you to the appropriate plan for your situation.
