Highmark WV offers plans for businesses from 2 employees on up.  They offer a variety of plans with varying deductibles, coinsurance percentages, prescription drug plans, and office copays.  Click on the link below to see all of their small group offerings.  Groups over 50 may custom-design their own plan.

To get quotes for a group, the group must complete a few simple steps.  Each employee that wants to be included in the plan must fill out a universal medical appraisal form, and must complete all information on the three page form.  Any employee who wishes to waive coverage must complete the first page of the form and sign at the bottom in the waiver section.  Highmark requires that the group also submit a Group Application and the most recent tax and wage/unemployment report.  

Once we receive these items, we will submit the group to Highmark for a firm, underwritten quote.  This typically takes 1-3 weeks, depending on how complete the information is.
